1. This a solicitation for a commercial item prepared in accordance with the format in FAR Part 12, Acquisition of Commercial Items in conjunction with FAR Part 15, Contracting by Negotiation,
supplemented with additional information included in this notice. This announcement constitutes as a solicitation and proposals are being requested.
2. The Government intends to award a Firm Fixed Priced (FFP) contract, with a 5 year Period of
Performance (PoP) from 23 March 2020 - 22 September 2025.
Base Period: 23 MAR 2020 - 22 MAR 2021 (12 Months)
Option Year I:
23 MAR 2021 - 22 MAR 2022 (12 Months)
Option Year II:
23 MAR 2022 - 22 MAR 2023 (12 Months)
Option Year III: 23 MAR 2023 - 22 MAR 2024 (12 Months)
Option Year IV: 23 MAR 2024 - 22 MAR 2025 (12 Months)
Six Month Extension: 23 MAR 2025 - 22 SEP 2025
3. In order to be considered for award, offerors must adhere to all requirements detailed
herein. Submit written proposals in reference to Request for Proposal (RFP) reference number FA4890-18-R-5035.This solicitation is being issued as a 100% Small Business Set-Aside. The NAICS code for this requirement is 621340 - Offices of Physical, Occupational, and Speech Therapists, and Audiologists.
4. Description: The objective of this requirement is intended to support the OHWS program through prehabilitation to decrease the impact of neck and back pain.
Decreasing neck and back pain will have a positive impact on aircrew retention.
5. Evaluation: The Government intends to utilize a full tradeoff evaluation methodology that meets all the requirements listed in the Evaluation - Commercial Items (52.212-2), Attachment 2.
6. To be considered for award, you or your company must be registered with SAM at https://www.sam.gov/portal/public/SAM/. Please note that electronic funds transfer through WAWF is the only acceptable means of invoicing and payment. Refer to https://wawf.eb.mil for registration information. Offerors must provide their DUNS number, CAGE code, and confirmation of their registration along with their proposal
7. The Government will accept questions regarding proposal until 23 August 2019. Responses to the questions will be posted for all to view. Proposals are required not later than 10:00 a.m. local (Eastern) on 16 September 2019. The Contracting Officer is Capt Matthew Markling, (757) 225-2838 and the Contract Manager is LaTisha Ross at (757) 764-9435.
