Scope of Work The City of Ormond Beach, Florida is seeking proposals from qualified Health & Welfare insurance carriers to provide Employee Group Health and Vision Insurance Benefits. The effective date of any plan adopted will be January 1, 2017. The current health plan offers a fully insured HSA and POS option. Please offer medical quotes with the following options: matching current plan designs, matching 2014-2015

plan designs, HSA plans matching current plan design but with an embedded deductible, Non-HSA HMO buy up plans, level-funded and self-funded plans. The City is also seeking proposals for fully insured Vision insurance with coverage matching current benefits and coverage with richer benefits, including higher reimbursements.
Sealed proposals for Employee Group Health Insurance, RFP Number 2016-32, will be received at the Office of the Purchasing Coordinator, Ormond Beach City Hall, until 1:00 PM on Tuesday, August 30, 2016, at which time they will be publicly opened in the 2nd Floor Conference Room, located in City Hall, and the names of the firms or individuals submitting proposals will be read aloud. Submittals received after the time and date specified above will not be considered.
