Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The equipment being requested to expand our kiosk use in the new central registration area and replace a unit on the 6th floor. The kiosks

allow patients to do in-person demographic/insurance verification, check in for appointments and apply for travel pay and release of information requests. This provides faster service for patients and more confidentiality The current kiosks operate on a hospital wide software program. The additional kiosks will need to be on the same program as other Kiosks currently utilize at the facility. This will allow for seamless operation, maintenance and Standardization of Kiosks already in use. Software program is proprietary to VECNA VKiosk® Kiosks. Description of market research conducted and results or statement why it was not conducted: This is a brand name justification in support of FAR 11.105, Items Peculiar to One Manufacturer. Specifically, only VECNA Technologies brand name. Based on market research, it was determined that limited competition is viable among authorized resellers for this brand name item. Product is available on the Open Market as Small Business Set Aside. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief. Note: COs are required to make a determination of price reasonableness IAW FAR 13.106-3.
