Due: 12 Apr, 2024 (in 14 days)Agency: Finance - Purchasing SCOPE OF WORK The City of Downey is requesting written proposals from qualified homeless service providers to provide outreach, emergency and interim housing options, and housing navigation services for homeless individuals and families in the Downey community for a period of one year. The professional services provider will be required to perform the following:  Provide and manage two outreach workers to engage and assess the needs of homeless individuals in the Downey area and connect them with, or provide referrals to, housing services and resources in Downey, Service Planning Area (SPA) 7, and/or the greater Los Angeles area.  Use Service Prioritization Decision Assistance Tools to link individuals with the services they need in order to acquire permanent housing and increase and maintain health and well-being, including interim housing, mental health care, medical care, substance abuse treatment, wellness/life skills programs, and income assistance.  Work with Los Angeles County and other service providers to ensure emergency and interim housing resources are readily accessible to homeless individuals from Downey in accordance with the agency’s normal practices, policies, and procedures. Transition individuals into interim and permanent supportive housing as quickly as possible.  Make available all permanent housing options appropriate for each person, including but not limited to housing placement through the SPA 7 Coordinated Entry System, Los Angeles County Department of Health Services Housing for Health (HFH), housing through the Los Angeles County Department of Mental Health, Housing Authority of the County of Los Angeles (HACoLA), licensed care facilities, and reunification with family and friends in other communities.  Work to ensure that all veterans who are found to be among the homeless population in the Downey area will be connected as swiftly as is reasonably practicable with housing and support services funded through the Veterans Administration and other entities providing resources and services for veterans.  Provide a monthly report that includes information about the number of contacts, repeat Page 4 of 33 contacts, and clients that have been placed in temporary, interim or permanent housing, and use this data to inform strategic decision-making, demonstrate program success, and highlight homelessness service efforts. The report and other findings will be discussed during regularly scheduled monthly meetings with City staff and appropriate partners.  Participate in community meetings and events related to homelessness to provide insight and gain knowledge about areas and individuals that could require assistance.  Be available to respond to calls from the Downey community about specific homeless individuals during normal working hours, generally Monday through Friday, from 8:00 AM to 5:00 PM.  The team will also work closely with the City’s Homelessness Solutions Manager, Downey Police and Fire Departments, and the Community Development Department – Code Enforcement Division to help ensure smooth coordination and referral for clients who make contact with public safety or emergency personnel.
